For women who've experienced childbirth, vaginal muscles tend to expand during the delivery, the result can often be weak, loose, vaginal muscles. It's the scenario that every woman dreads. If she asks her partner if it "feels the same" as before she had the baby he'll nod, unwilling to tell her that, urm, well, it's not as it used to be. A comment like that would be tantamount to saying her bum looks big. He just daren't go there.

I was that woman, after giving birth to a super 8 lb baby girl, no stitches; I was thrilled that I quickly managed to regain my figure. My breasts were firm and rounded, engorged with milk. No one ever tells you how incredibly sexy large breasts feel and as an AA Cup holder, it felt lovely to sport the mega Gold Medal versions.

We both couldn't wait to get reconnected - we hadn't made love during the last few months of my pregnancy and to be quite honest we were both gagging. Much to my dismay what I found was it that sex was less than satisfying, it just didn't feel the same, my vagina felt lose, I had lost a lot of sensation and if I sneezed or laughed I might also leak a small amount of wee. I asked my partner how my vagina felt during lovemaking, and if there was any difference - after much reluctance he said that it didn't feel as tight, he said it felt rather lose and that he was struggling to get the sensation that he needed to keep his erection.

With military precision we both approached my/our vaginal tightening debate. I read up about Vaginoplasty a procedure where part of the vaginal wall is cut away and the excess vaginal lining taken away. This tightens vaginal muscles and surrounding soft tissues. Although this is known as a "standard" gynaecological procedure, it is still an operation under anaesthetic, there is scarring and, I felt a rather drastic step - that one would have to be a last resort. My friendly GP cautioned me about the op, he said that I could lose sensation and was rather puzzled why I wanted a tighter vagina. I felt he didn't quite get it. He referred me to a Women's Health Clinic, where a physiotherapist taught me how to do manual pelvic floor exercises properly. She tested the strength of my pelvic floor using biofeedback. The proof was there, it was very weak and she said if I had not started a strengthening programme for my pelvic floor with pelvic floor exercises then I would be in danger of developing bladder weakness and possibly prolapse. My mum had suffered a vaginal prolapse after I was born, and was unable to have further children, I didn't want history to repeat itself and we had planned to have more children, not just the one. I also learned that a weak pelvic floor is hereditary - like mother like daughter or so they say. This was one likeness I was determined not to share.

So with genetics against me I set about my pelvic exercise regimen. Strange as it is, even though I was faced with the fact that I might never be able to have another baby, I just didn't do the exercises. They were tedious, boring and repetitive.

When I went back to my Physio for a review, I knew I had to be honest with her, its nothing she hasn't heard before and she looked at me rather exasperated. I knew how she felt, I felt it too. It was then she said that if I wanted the easy solution, then I could try one of the new breed of pelvic exercise machines. She showed me one that automatically does the squeeze and release exercises for me, I was sold on that idea. My new exercise routine was sorted, I could feed my baby and have my pelvic floor exercised at the same time, and boy I could feel it like never before!

My partner noticed my tight vagina before I did, within 2 weeks he was commenting about the improved tightness and my orgasms started getting easier and easier.

I have used my Kegel8 for over 2 years now; my pelvic health is at an all-time high. I can make my partner orgasm just through clenching my vaginal muscles; I couldn't do that before baby! Urinary incontinence (UI) means the unintentional leakage of urine. It is often termed 'bladder weakness' which is a misnomer as it rarely involves a problem with the bladder at all.

It remains a 'taboo' subject that is not openly discussed even with a partner; less so with a doctor, other health professional or friend. Stress incontinence is the most common form, and is caused by physical exertion such as exercising, laughing or sneezing. Urge incontinence is another form where the woman feels a strong urge to urinate and cannot stop herself. (NICE 2006) Whatever the type: it is distressing. The situation is only managed by the wearing of pads at considerable cost to the pocket and, more importantly, self-esteem.

Antenatal advice, mainly given by midwives, is to regularly practice pelvic floor exercises, otherwise known as Kegel exercises to avoid the leakage of urine occurring. In a study summarised by Sue Dwyer (2006) of 3204 women, over a third of women practiced pelvic floor exercises from the beginning of their pregnancy to two years after the birth.

Why is it then that almost half these women still suffered with UI?

Possibly, the exercises are being performed incorrectly due to a lack of adequate initial supervision when identifying the correct group of muscles or not often enough. One study states it is necessary to exercise these muscles 80-100 times each day (Glazener, Herbison, Wilson, MacArthur, Lang, Gee, Grant 2001). Even on a more reasonable level it should be 8-12 times three times daily (Jozwik 2007) preferably in each position of lying, sitting and standing.

So, what else could be done?

Studies have shown that an intensive programme of pelvic floor exercises overseen by a qualified physiotherapist (Chiarelli & Cockburn, 2002) or specialist nurse (Glazener et al, 2001) can help to reduce UI. The problem is that with resources scarce and exercises requiring great motivation women tend to become frustrated with the lack of long term results. Pelvic floor surgery is an alternative and is often chosen as a speedy answer to the problem but it can reoccur as the muscles weaken again.

How about a solution that allows you to take control?

It is three times more prevalent in women than men, largely due to it being a common condition during pregnancy and women having smaller wrists.

Carpal tunnel syndrome is caused by compression of the medial nerve. This nerve passes through the carpal tunnel - a space in the wrist formed by the carpal bones and the flexor retinaculum (or transverse carpal ligament). The tendons of flexor digitorum superficialis and flexor pollicis longus also pass through this space.

Causes of a decrease in space in the tunnel include traumatic wrist injuries, repetitive wrist movements,fluid retention during pregnancy and arthritis.

The symptoms of carpal tunnel syndrome include:

A dull ache in the wrist and forearm; Pain which radiates into the thumb and fingers (excluding the little finger); Sensations of tingling or burning in the hand or fingers; Pain which is worse at night; Pain may radiate into the forearm, elbow or shoulder; Weakness in the fingers and hands.

There are also a couple of quick and easy tests which can be used in the diagnosis of carpal tunnel syndrome. Tinels sign involves tapping over the carpal tunnel. A positive test reproduces the symptoms. Phalens test is where the patient places their hands at chest height with the finger tips of the two hands touching. They should flex the wrists so that the backs of the hands are together. Hold this position for a minute. Reproduction of any symptoms is a positive result.

There are many treatments which have been recommended for use in carpal tunnel syndrome. Generally it is treated conservatively with rest, splinting, anti-inflammatories and cold therapy, followed by a rehabilitation program once symptoms decrease. Persistent cases may require a surgical procedure known as a carpal tunnel release.

In certain circumstances, a C-section is scheduled in advance. In others, it's done in response to an unforeseen complication. The unexpected problems happen during delivery are as follows:

• Health problems in mother
• Position of the baby
• Not enough room for the baby to go through vagina
• Signs of distress in baby
• Labor is slow and hard or stops completely
• A problem with the placenta or umbilical cord puts the baby at risk

It is most common to be seen among those mothers who are carrying more than one baby. The surgery is relatively safe for the mother and the baby, but still it is considered to be the major surgery and carries risks. It also takes longer to recover as compared to the normal vaginal delivery.

Cesarean sections cause many complications; women who have c-sections are more likely to have an infection, excessive bleeding, blood clots, more postpartum pain, a longer hospital stay, and a significantly longer recovery. Injuries to the bladder or bowel, although very rare, are also more common. In addition, if one plans to have more children, each c-section would increase the future risk of these complications as well as placenta previa and placenta accreta. Not all c-sections can or should be prevented, as in some situations, a cesarean section is necessary for the well-being of the mother, the baby, or both.

In the past 40 years, the rate of cesarean deliveries has jumped from about 1 out of 20 births to about 1 out of 4 births. This trend has caused experts to worry that C-section is being done more often than it is needed. Because of the risks, experts feel that C-section should only be done for medical reasons.

The egg attaches to the womb and begins growing. But in an ectopic pregnancy (also known as an extrauterine pregnancy), a fertilized egg implants in another place but not the womb, frequently in a fallopian tube. An ectopic pregnancy is often caused by damage to the fallopian tubes. An impregnated egg can have difficulty in travelling through a damaged tube causing the egg to attach and grow in the tube. Rarely, the egg implants in an ovary, the cervix or the belly. If the egg keeps growing in the damaged fallopian tube, it can damage or burst the tube and lead to heavy bleeding that could be lethal. If you think you have an extrauterine pregnancy, you need prompt treatment to stop it before it leads to severe health problems.
Factors bringing on salpinges damage
1. Smoking. The more cigarettes a day you smoke, the higher your risk of an ectopic pregnancy.

Treatment options
Treatment of an extrauterine pregnancy is surgery, frequently by laparoscopy these days, to remove the ectopic pregnancy. A ruptured Oviduct usually has to be removed. If the tube has not ruptured yet, it may be possible to fix it.
Future pregnancies
The extent of the operation impacts upon the chance of sucessful future pregnancies. If the Oviduct has been fixed, the chance of a successful pregnancy is commonly higher than 50%. If a uterine tube has been excised, an egg can be fertilized in the other tube, and the probability of a successful pregnancy is lower than 50%.

The 36-year study, which tracked thousands of women, discovered that women who took fertility drugs developed thyroid cancer at an increased rate over those who did not. Compounded with the previously documented risks of multiple births and other complications, many couples have begun seeking natural alternatives to the use of fertility drugs.

"Some new drug-free programs avoid these risks," said physical therapist Belinda Wurn, who developed a natural fertility treatment with her therapist husband. The treatment, a unique manual therapy, underwent clinical trials, and results have been reported in several medical journals, including Medscape General Medicine Ob/Gyn & Women's Health.

In the first study, 71 percent of women diagnosed infertile by their doctors an average of five years, became pregnant naturally within one year of receiving the therapy. Most of the women gave birth, and several have had subsequent natural full-term pregnancies. While the total number of participants was small, the results were considered important enough to be published in a major peer-reviewed medical journal.

In another Medscape study, scientists examined the effectiveness of using the therapy before in vitro-fertilization (IVF) in women averaging five years of infertility. Those who had therapy before IVF transfer reported a clinical pregnancy rate of 67 percent. The control group (no therapy) had a 41 percent rate - the national average for clinical pregnancy after embryo transfer according to data from the US Centers for Disease Control (CDC) and the American Society of Reproductive Medicine (ASRM). Most of the pregnancies in the therapy group reported live births.

A study published in Alternative Therapies in Health and Medicine showed that therapy opened totally blocked fallopian tubes, a procedure previously thought impossible without surgery. Of the women whose tubes opened (61 percent), most went on to have natural pregnancies and births.

Patients who came for treatment of chronic pain would often report dramatic improvement in seemingly unrelated areas, such as digestion, elimination, and reproductive or sexual function."
